You probably have the game on both NAND and USB, which causes the USB game to take over once the console is booted up.

Mine is a 5.5.5 USA. Probably nota compatible with all consoles, as you said. On a Brazilian game forum at least 3 more users had the same error.Got exactly the same issue. And I noticed in step3, it showed "patched 0 entries". I guess that's the main reason.
Took some time checked the fst file and fst patch code. Seems my cosoles's fst file doesn't have a valid inject pointer.
So this should not be a general hax for all consoles. At least 3 people in this thread have the same issue.

Already tried that, and sadly got the same error.Try copying the files to the sd card again, verify everything is in place and file sizes are correct
