Grab the latest TEMP_USRCHEAT.DAT.zip.
If you're using AKAIO, then use the built-in WiFi updater (be sure to choose Temp as the Cheat DB). If you don't have WiFi handy, grab the above file from http://cheats.gbatemp.net/Temp/ and unzip it into _aio/cheats on your SD card.

You can't exactly unpatch a rom, but you can always just redownload or redump the rom, wherever you got it from.
Actually, many patch files out there generate a ".bak" file, which is actually the original unpatched rom with a different file extension, if you kept that somewhere rename it to .nds.
