Just do as below.(Original Post)
You gotta clear your icon cache. You have to delete CacheD.dat located at "SD:\Nintendo 3DS\[ID1]\[ID2]\extdata\00000000\0000008f\00000000\00000005"
Note, I'm assuming your console is North American. If EUR, the 0000008f folder is 00000098, and if JPN it's 00000082. In any of those folders, you wanna delete the file "00000005".
Please also note that this'll remove your current theme, but won't actually delete any themes. It won't wipe shuffle theme preference either, you can go right back and set it to what it was, if anything.

What else does that file store other than icons/theme info?

Also it appears that this doesnt clear it out of the Activity Log... still shows up as FBI in there... is that cached elsewhere? How do I fix that?

Does it ever rebuild the icon cache on its own? Im fine wiht it if it eventually will re-cache the correct icon?

Also the intention is not to wipe it completely but to either wipe a few titles or edit the icon to point to correct one.

editing the activity log would be nice... :) i read someone ages ago trying but he didn't have any luck, but as i said it must be possible.
maybe the format is documented on 3dbrew

I used FBI to completely delete the Health and Safety app. Then I rebooted the 3DS and installed it back on. That cleared the icon up for me.

Did you dump it and then uninstll then reinstall your own dump? Also are you talking about icon in activity log? or icon for the app?

I used a fresh download from 3DNUS. As for the Activity Log, I don't care about keeping track of my Health and Safety activity, so I just removed it from Activity Log.

I'm sorry that's happening for you. After restoring my icon and removing Health & Safety from the Activity Log, the next time I launched the app and then checked my Activity Log, it had the normal icon again.

How did you find the Title ID to download it? Seriously system Title IDs dont seem to be documented anywhere.. only the games.

Edit, finally tracked it down.. for N3DS its 0004001020021300

However, removing it with FBI, then rebooting, then reinstalling... didnt fix the icon in activity log.

I'm planning to start building it back up though. Plus, Usenet automation around music downloading has gotten so much better since then

I used to use high end headphones and speakers JBL back when they made good speakers lol X-Fi Fatality edition sound card on PCI with XP back when Windows had good sound....

I mean built in sound on mobo's has gotten way better but still XP handled sound better and X-Fi was still better than onboard audio even to this day

Hell not sure what was going on but for like a few weeks MP3's sounded muffled, some driver or Windows update fixed it. Thank god lol

Oh boy Massdrop has $1100 IEMs. Want, but not at that price lmao. https://drop.com/buy/campfire-audio-andromeda-emerald-sea-iem

I'd sooner buy the Meze 109 Pro if I was dropping that much on headphones. I don't even like buds/IEMs
  • Sicklyboy @ Sicklyboy:
I got the Google Pixel Pro buds, they're good enough for when I need portable audio. And some really cheap IEMs that Dankpods recommends, I think the KZ ZSN Pro

I'd stick with cheapo Chinese $10 ones quality is actually becoming on pair with name brand since name Brand usually quads the price up anyway loose one bud that's another $200
  • Sicklyboy @ Sicklyboy:
My Pixel Pro buds shit the bed a month or two ago. My wireless charger (which they do support Qi charging) absolutely COOKED them. Caused some problem that caused the case to heat up to 180F+
  • Sicklyboy @ Sicklyboy:
They were like 9 months out of warranty but I hit up Google support anyway and told them how hot they were getting and they replaced them with new ones because they wanted these for failure analysis lol
  • Psionic Roshambo @ Psionic Roshambo:
    lol wireless charging
  • K3Nv2 @ K3Nv2:
Yeah that's why I can't bring myself to even spend $100 on wireless earbuds
  • Psionic Roshambo @ Psionic Roshambo:
I think it's a cool idea but damn
  • Sicklyboy @ Sicklyboy:
Apparently the case has a problem with >15W wireless chargers. Google design fault, that one.
  • K3Nv2 @ K3Nv2:
I could live with wireless charging if the damn standard case doesn't block connection
  • Xdqwerty @ Xdqwerty:
    how is wireless charging possible?

    Copper conducting electricity or something
  • Sicklyboy @ Sicklyboy:
@Xdqwerty, power through a coil of wire causes an electromagnetic field to be generated. Another coil of wire can be set up to harness the power from that electromagnetic field and turn it into usable energy for charging a device
