thanks! You gotta clear your icon cache. You have to delete CacheD.dat located at "SD:\Nintendo 3DS\[ID1]\[ID2]\extdata\00000000\0000008f\00000000\00000005"
Note, I'm assuming your console is North American. If EUR, the 0000008f folder is 00000098, and if JPN it's 00000082. In any of those folders, you wanna delete the file "00000005".
Please also note that this'll remove your current theme, but won't actually delete any themes. It won't wipe shuffle theme preference either, you can go right back and set it to what it was, if anything.
thxYou also can dump Cache.dat with https://github.com/MrCheeze/extdata_dump, modify it with some hex editor, setting to 00s and FFs the line of H&S cache and inject the modified file. Of course is more complicated, but you won't lose any setting.

Sorry for the late response, but I haven't been able to figure out the Activity Log icon cache. As for what else is stored in CacheD.dat (00000005), nothing else is stored in it as far as I know. I think the reason theme gets reset to default is because there's another file that depends on CacheD.dat, but I'm not entirely sure of this either. There's a lot of info about this on 3DBrew.2 Questions for this.
1) What else is stored in that file other than theme info/icon info that can be regenerated?
2) Deleting it still doesnt clear the icon in activity log, is that stored elsewhere? How do I fix that one?
