Hacking USB Loader Covers

Ok, I have CIOS rev10 and SD/USB Loader 1.5.
Whenever I open the loader it shows "No cover" for my games.
Ive tried using the code it comes with when downloading and renaming to the games name but still it doesnt work.
It meets the spec.
Help?
