You don't need to. However, you need to delete TWL_FIRM patches if you have them.

You should. A clean sysnand is ideal

DSIware is not the problem, the only problem is if you have a patched TWL_FIRM which is basically the thing that plays the DSIWare. Be sure to install the original version of TWL_FIRM before continuing. Also once you have A9LH installed you don't have to patch it again since if you use Luma3DS it will patch it as well as the AGB_FIRM(GBA Mode).

I did the OTP guide with the custom TWL-Firm that allowed the original R4 to launch on 3ds. I never had any issues.
