I am currently using an R4 with nesDS. My punch out rom looks very glitchy and I can't see the health bar and what not. Do I just have a defective rom or is this one of the games that it isn't 100% compatible with. Thanks

NesDS cannot run Punch Out!! properly. The only way I've found to run Punch Out!! on the DS (without major graphics problems) is to use HVCA, but it's a GBA program, so you can't run it from the R4.
