i usually take a 5 minute break every half hour to an hour, at the moment i have zelda and street fighter, on zelda i have a break after every dungeon and only set the slider to somewhere between off and half way and on street fighter after each play through of arcade mode with the slider on full.
I also have glasses which i never needed for the ds as they was for when i was looking at a computer screen for 8 hours plus a day at my old job and i only just need them due to being slightly long sighted but i found by wearing them i never get any kind of headache or eye strain.
I think it's a trial and error thing for each person to get what's comfortable for them, some people may never get used to it.
