For me: Mega Man & Bass GBA had a horribly translated. And for SNES - Mega Man & Bass hacking translated was closer accurate but not perfectly translated.

The Breath of Fire games come to mind.
The GBA ports re-used the SNES translations without fixing any of the problems present, as I recall BoF1 has an instance of telling you to travel east when you actually needed to head west, or something.

Of course, there's the fan translation of the SNES version of Breath of Fire II that goes above and beyond on a technical level so that a proper script can be put in.
