Hardware Having problems with SD card

My microSD card is having some problems right now.
On my drive list, it shows up as a removable drive, with no filesize or anything: http://puu.sh/h1j28/698b5102e8.png
It can't be opened or formatted.
Is the SD card bad or can this be fixed?
Also, trying with an SD adapter and a USB adapter gave me the same results.

They're a few possibilities for this to happen.
One, is that your USB adapter and other reader/writers is not SDHC or SDXC compliant.
Two, it could be that your SD or MicroSD is dead.
Three, bad contact pins on your SD, MicroSD, Card readers, etc.
Four, it could be that you may need to uninstall your SD reader/writer and USB adapter drivers from windows and reinstall it using Windows drivers if any. Best to try it on another computer if available first.
If it is actually number two, then you are SOL, you will need to find a replacement for it. There maybe more possible scenario that I may have not thought of, however the four scenarios listed is most likely is the cause.
