im pretty sure that the 3ds will not show anything when any card is set to exfat. It needs to be fat32 for the 3ds to read the card.

trust me, ive used the skydock to extract pkmn us and um.
i know how to inject my own private header into a rom and change its chip id to match.
i always use the 4gb versions of my games on my sky3ds+ dual orange button with exfat
on fw v140
you also have to use encrypted games in .3ds format.
cia isnt supported, nor eshop games.
try reformatting your sd card and put an encrypted.3ds game on there

First of all, update your sky3DS+ firmware to V140:

Copy the firmware.bin of sky3ds+ firmware V140 into the root directory of your FAT32 microSD card. Insert your microSD card into sky3DS+, connect your PC and sky3DS+ with a USB cable. The LED red light will turn on for 30 seconds, then turn to the green light and keep flashing for 10 seconds, once the light turn off it means the sky3DS+ has been updated successfully. Delete the firmware.bin file in your microSD card. then format the sd card to exfat 64kb for supporting games over 4gb
if all else fails, re-download that rom from more reliable sources, make sure it is not out of region, or not a modified rom.
