Hacking What should i type in here?

I am about to buy a DSTwo on NDSGamer.com and i have to make an account and type in my address first.

in the address portion i am confused a bit. here is the screen shot

for "Street" portion... why is there a small box next to a big one? i dunno how to type in my address.
for a form like this how would i type my address which is:
104-56 63rd ave. Apt. 2C
i dont wanna mis-enter these and not get my package!

It seems to me that you would fill it out this way:
Building name, apartment, etc. | 2C |
Street | 104-56 |

| 63rd Ave. |
