Recently ran into this problem
I installed inputredirection.cia accidentally when i wanted inputredirectionNTR.cia
should not have been a problem, delete and install the right one
but, i saw that inputredirection.cia actually replaced the cia of persona Q shadow of the labyrinth
i used fbi and 3dsdb and found that the two titleids were exactly the same

is this a common thing? if so, how do i avoid it?

Whoever made inputredirection.cia didn't pick a properly unique ID when building, you should contact the author and let them know they have a conflict. There's unfortunately nothing you can do from your end about this unless you want to unpack/repack the CIA with a new ID yourself, but it's probably not worth the trouble.
