Here's one that bothers me. When I do a Save-Shutdown with Firefox then turn it back on if any of the windows was YouTube the video will start playing. This is compounded by the number of YouTube windows opened. Imagine ten videos running at the same time.
10 videos at once? try that + 30 other tabs killing my slow internet,
but now it doesn't matter that I have over 100 tabs.
Try the Firefox addon Bartab lite. It makes firefox only open the tab you were on, and then the other tabs when you click on them, but I wish the original was updated, because it let me unload tabs & free memory.
what annoys me?

No!!
Are you sure you want to leave?
Really??
Ok choose leave page to stay on the page.
Let me go! I want out.
Several options here-
I am not sure if there is a bulletproof solution on
http://www.macromedia.com/support/document..._manager03.html somewhere but if you have not seen that before do have a look.
Noscript and other such things can be set to block flash objects until you click on them
Combined with noscript above I use
https://addons.mozilla.org/en-US/firefox/addon/greasemonkey/ and
http://userscripts.org/scripts/show/13333 and tell it to stop autoplay, autodownload/autobuffer.
Re ads: if an advert peddler makes it past my layers of blocking and security (off the top of my head adblock+ lists, custom hosts file, ghostery, noscript, Blitzableiter, greasemonkey + scripts, general config things like block redirects, taco (or I suppose beef taco these days) and that is just my general walking around browser) I will not pay it any attention or might pop up firebug to lose it but a silent nod will be sent in their direction. Hopefully the day will come soon when either flash fails or gnash works well enough.
Anyhow I will echo the sites that almost completely fail to function without javascript. I absolutely accept reduced functionality- ignoring HTML5 for a moment javascript is good stuff and there are probably even justifiable reasons for having it required at some point (part of security or something) but when news sites (see gawker media sites) and sites like
http://marketplace.xbox.com/en-US/Games/Xbox360Games can not even get it done then.....
On a similar note when flash videos fail to work when you block local storage.
