Cross Treasures is a very good game, you can play it without knowing japanese, for those that just dont care about the language, and for those who care... there's an open translation for this game here in gbatemp at hacking and translations section.

what about 'Pic Pic'

It is a great puzzler but always ignored.

LOL is quite good IF you have good ideas about what to do and are playing with 4 people. However from what I've read it didn't sell very well

Ahh, I love age of empires:age of kings.
Idk why, It is super addicting, I would sit for hours in free play trying to defeat others castles and farms.

Too many responses so I couldn't check them all but...

Nodame Cantabile? I like the anime, but I've never played the DS game. Can anyone confirm it's an actual playable game? Do I need to understand Japanese?

The only reason I ask is because a lot of games based on an TV or Anime series are horrible games with an interesting story, made for fans to spend more time with their favorite characters. Since I wouldn't be able to understand any of the Japanese...it'd be completely pointless.

"Disney's A Christmas Carol" - a decent point & click adventure game. It's based on a movie, but the movie based on a Charles Dickens' book, so it got a good story.

Gameplay is good, too. Beside the point & click part, you'll have to pass some mini games with touch screen. You can play them again in Collections to get higher score. And you can unlock several images for "spot the different" mini game when Christmas(of 2009) come.

The game has the original book of Charles Dickens, and a built-in reader so you can use DS like a digital book

Nice list and all, but I don't see how this works out.
All these games can be considered underrated, yet everything is opinionated.
Looking at the list, I can tell before even trying out the games that I would not enjoy any of them other than maybe 1 or 2 games.
Other than that, personally I feel like phoenix wright series is underrated, yet they got around 8.0/10 score, so is that considered under rated?
So yeah, nice list and all, but I don't think "under rated" is the best term to be describing this.

+1 Rooms: The Main Building

I'm not the biggest puzzle-game fan, but I checked this one out on a whim, and man, what a nice surprise it was. If you liked Exit DS, you'll probably dig this one, too.
