Usually, I had issues with wrong backgrounds and covers size caused by incorrect platform.ini/magic numbers, or by out of memory errors (when I was using non optimized png's for my theme and covers).
I remember one time I accidentally left two magic numbers in one of my source_menu.ini button, they were for plugins with different cover size, and it ended displaying everything with the standard DVD case format used for Wii games.
I even had a recent issue with SD card corruption, or so I believe.
In my case it was a serious series of issues (not even Saitama could do more damage). Everytime I launched the WiiXL emulator in Wiiflow, it resulted in a black screen. I had to manually reboot the console keeping the power button pressed. On reboot, it completely wiped up the ENTIRE apps folder on the SD. I copied back the folder from my pc, then tried again, the next time it deleted the entire apps AND Wiflow folders from the SD root! This happened only with WiiXL, everything else was working good.
I then used a brand new SD card, with an exact copy of all the files.
WiiXL was perfectly working, and so everything else, so I can only think about file corruption
