Re-install the firmware, and use Panasonic SD formatter.

That means it doesn't support v1.35...
You should find and download v1.34.
Or even better, get an Acekard2i or a SupercardDS2.

Try YSMenu maybe, or Wood R4. Either that of your R4 or SD card has something wrong with it. Backup and format the SD. Download one of the two kernels I mentioned, or the official again, and see if it works. If it doesn't maybe it's broken?
