You can either...

A) Beat the game on maximum difficulty
B) Go to the War Room and enter this sequence: Select, L, R, Select, Y, Y, X, R, L, X, R, Y. You have to re-enter it every time you want to play, though.

I know two people (besides myself) who have legit copies of the game, beat the game on maximum difficulty, and still did not get survival mode. Does it work for anyone?

No it's after hardened.
