ROM Hack How far can one go?

How far can one go with hacking a ROM? I understand you can edit palettes and placement of objects, but can you go as far as editing and inserting your own graphics as well? And if so, couldn't one hypothetically fill the entire ROM with ones own creations, thus creating a Homebrew that is greater than 4 Mb?

You...could fill the ROM with your own creations, but you're still using copywritten code. It's not homebrew then, it's a hacked commercial game.

If you were to rewrite the game code from the ground up, then it's not ROM hacking, that's writing homebrew. :/
