Help I'm looking for XBMC 3D cover template

I want to create custom/missing 3D covers for XBMC, using existing format. I cannot find template or instructions. Your help is greatly appreciated.

I found this on my hard drive. It's an action file that you can run in photoshop over an existing cover. I do not know how to use it properly, though.
