Do you play every new game on hardest difficulty?

Medium/Normal for life. It's usually the difficulty that has been balanced the most, and often the higher difficulties just give enemies more attack and more health. If I really like a game and want to replay it, I might go for hard, but it's very rare.

Depends on my familiarity with the game in question's mechanics. For a series I'm skilled enough to play mindlessly (or which will just require a bit more grinding to beat at a higher difficulty), I might pick hard, but my default option for new games is Normal.

Depends what I am trying to get from the game, if it's trophies then I put it on what is required. If it's a game I want to enjoy it's medium and if it's call of duty it's veteran (I can't be bothered with realistic).

No, I almost always use only medium/easy (not that most games I have are adjustable, indeed)

but i'm not playing on hard just to make things unnecessarily annoying.

It all depends on the game but most I play it's on hard.

I always play on hard, because trophy/achievement. I rarely replay game.
