There's also the Supercard DStwo(Plus), though it's not cheap. The biggest benefit is built in GBA emulation, so you can even use it on the DSi, and not need to install a CIA file.

Is R4 still the best or is there something better or cheaper out now?b
SuperCard DSTWO is the best NDS flashcart on the market, but it does cost more than the others due to the onboard CPU (this is used for GBA and SNES emulation but you can do those in 3DS mode with homebrew too if you buy Freakyforms Deluxe)
It has the most robust cheat system and Real Time Save function as well, with a built in cheat searcher.
The original DSTWO cart is no longer available so you will have to buy the more expensive DSTWO+ (which has a faster CPU and 3DS ROM support but I would not use the 3DS ROM support as it is known to brick and you would need to be on 9.2 or below anyway)

But if you're not willing to pay that much for a flashcart, this cart is the second best and it will do just fine for playing DS games: http://www.nds-card.com/ProShow.asp?ProID=149

It does not have a faster CPU, it has more CPLD and flash (for...?) Supposedly, the official menu and TempGBA use them for something which makes it faster somehow.

Since the DSTwo and DSTwo+ are the same price, you might as well get the newer one though in case it somehow is faster.

I got the R4 Dual Core 2016.

While it may be the worst flash cart I've ever owned, for 10 bucks, it plays NDS Ron's adequately, and if that's all you want it's a decent option for the price.

How do you patch it to make it compatible?

Nintendo unsigned quite a few signatures to run the NDS flashcards in the past. With Custom firmware we can sign the signatures again so the flashcards work again. This does require either menuhax+Emunand or A9LH+sysnand both with CFW ofcourse.
