Possible bad conversion even if icons are not correctly converted the WiiU will see it as corrupt.

That why it good to install via USB rather than NAND things could get badly Corrupted and possibly spread through other Blocks in the NAND.

So I've tried this injector with Phantasy Star Online Episode 1&2, and I believe I followed all the instructions but when I install the game it just says "???" as the title and shoots an error about corrupted data, is it a bad conversion, a bad iso, is it because i installed it to the Wiiu's NAND instead of an external vWii drive.....or something like that....

Don't use "&" for the game description, the WiiU doesnt like this.
If you want to use "&", you need to use "&".

Example:
Phantasy Star Online Episode 1&2

Well, removing the ampersand did work, but now something else came up. The game installed, and starts but when it asks me to set the clock pressing yes just results in a black screen, requiring me to restart the system

Nothing happens

Try one of these steps which I normally give out to people who have issues with GamePad for some games:

#1 didn't work for you so try #2

Classic Controller Resync #1:
In game Classic Controller must be resynced by pressing the Home button on the GamePad and then pressing the Home button again to exit back into the game.

Classic Controller Resync #2:
- Press the HOME button on GamePad to get to the Home Menu.
- Hold POWER button on GamePad until the GamePad turns off, this will only turn off the GamePad not the WiiU console.
- Wait a few seconds, then press the POWER button on the GamePad to turn it back on.
- Press any button on the GamePad a few times to re sync.
- If successful, you will see the hand/finger pointer on the screen which you can move with the left analogue stick.
- Press the HOME button on the GamePad to return to the game.
- GamePad should now emulate the Classic Controller.

The problem is that the home button doesn’t even work when I’m on the error screen. I’m forced to shut down

There's something wrong with your inject. I just tried it on mine pressed home button twice and it works with gamepad.

Make sure you're injecting the U.S. version ISO if you have a U.S. region Wii U or it's Europe Wii U do the Europe ISO. If it's out of region it has to be patched otherwise it won't work properly.

On the injector itself before injecting on the gamepad/meta options choose Force Classic Controller Connected option.

I’m using a US version with a US Wii U and I did tried both the regular and force controller options

Select your sd card on top, click Wii U widescreen, click generate Nintendont config file (nincfg.bin) done. Or it might be Force Widescreen I'm not sure, but the point is you generate the nincfg.bin on your sd card and it applies it to your Gamecube injects.

Can this be done for the USB option somehow?

I don't know what you mean by "usb option" if you mean for these injects? Yes, if you mean for something else explain. The injects can read the settings from your sd card when nincfg.bin goes and apply whatever settings you choose.

Yes, even though they're installed to usb they read off the sd card the nincfg.bin which is the settings for Nintendont. You don't install the settings to the usb drive because you can't it's formatted to the Wii U only.
