in surgeon you have 5 fingers, how the hell would you hold the wii mote putting 5 fingers in 5 buttons, without your hand hurt or having a very bad grip making you use both hands to grab it...

Crazy Idea but maybe holding it in a reversed way, like A,1,2 and D_pad pointing to the floor.
