As seen by the fact 3DS applications can be easily converted to CIA and back, the only difference is in byte D of the exheader (which is calculated by starting at 0, adding 1 if the code.bin is compressed, and/or adding 2 if the application will be installed to SD -- titles for internal storage and gamecards, thus including all cci/3ds, must have this 2 not added)
