first, a little presentation. I'm Vanack, and i have a Nintendo Wii PAL, softmodded, with USB Loader GX, which works great for most of my backup games (also WiiWare and VC games).
My games are stored in a USB external drive, formatted as FAT32.
I installed Waninkoko's d2x cIOS following these 2 guides:

It all worked as expected.
My situation is:
cIOS 249: d2x v10 beta52 base57 rev 65535
cIOS 250: d2x v10 beta52 base56 rev 65535
cIOS 251: d2x v10 beta53-alt base58 rev 65535
I also installed IOS236.
My USB Loader GX settings are:
Loader IOS: 58
Game IOS: 250
Some of my games require cIOS 249, so i select it into game options, and it's enough for it to run correctly.
Now, my troubles. I've got issues with VC Japanese games (Blazing Star for instance, which is installed on emuNAND, created dumping my NAND with USB Loader GX) and with Guitar Hero 5 (Guitar Hero World Tour and Guitar Hero III works great).
My questions are:
1) Is it correct to put 58 as Loader IOS? Should i put something else? For instance 57 or 56 if needed? How does they work? Is more important Loader IOS or Game IOS to make a game run correctly?
2) Is it possible to run Japanese VC games on a PAL system? I tried every display mode (force 50hz, force 60hz, force NTSC, force PAL480p, force NTSC480p, region patch, disc etc...) but the game shows a black screen. I read somewhere in this forum that if i set Japanese as main language of my system, it could work. Is that true? Some other advices?
3) Guitar Hero 5 shows a black screen. I tried cIOS 249, 250, 251, but nothing. Then i read here (https://gbatemp.net/threads/guitar-hero-5-black-screen.222635/) that it needs Hermes cIOS, so i installed them using this guide (https://sites.google.com/site/completesg/cios/hermes-cios) with Hermes Installer v4. My situation was:
cIOS 222: base IOS38 merged with 37
cIOS 223: base IOS38
cIOS 202: base IOS38 merged with 60
Then i tried changing Game IOS for Guitar Hero 5, and it didn't work neither with 222, 223 or 202.
Then i read here (https://wii.scenebeta.com/node/49973) that i could use Hermes Installer v5.1, so i used it and now my situation is:
cIOS 202: base IOS60
cIOS 222: base IOS38
cIOS 223: base IOS37
cIOS 224: base IOS57
Then i tried changing again Game IOS but none of them worked.
Then i realized that MAYBE i should put cIOS 222/223 as Loader IOS into USB Loader GX main settings. I did it, but unfortunately now the loader can't recognize my USB drive anymore. Is there any FAT32 compatibility issues with Hermes cIOS? It could work if only my USB drive would be recognized, but i don't know how to test it.
Any other advices?

UPDATE! The solutions are:
1) Guitar Hero 5: put the USB drive on port0, select the cIOS with base 56 (which, in my case, is cIOS 250), which is the only working for it. On port1, as Cyan said, it only works d2x cIOS r53, which is unstable and not compatible with many games.
2) Blazing Star (or i suppose any VC Japanese game): install on NAND or install on emuNAND and enable "Region Code Patch" in game options in USB Loader GX.
