Are there any major changes for users that already have a9lh installed?

slightly bigger payloads, as v2 gains some space.

Taken right from that link I gave you:
"The new version of arm9loaderhax (sometimes referred to as "v2" because it is installed using SafeA9LHInstaller v2 or FIRM81 because of its use of 8.1's firmware files to make room for larger payloads) is the latest version of AuroraWright's Fork of Delebile's initial implementation.

The advantages to updating at this time are very minor and only really ensure compatibility with future arm9loaderhax.bin files, and is as such completely optional."

will it work if i had installed arm9loaderhax.3dsx instead, and i am on a franken-a9lh that uses delebile stuff, except the stage2, that is from dark_samus3? (withouth OTP?)

Doesn't matter. The arm9loaderhax.3dsx you used installed the payloads to your NAND and that's it, doesn't change the end result AFAIK.

Doesn't that mean in the future we would have to upgrade to v2 for newer a9lh.bin files?

if the arm9loaderhax.bin starts to become bigger, yes.

Yeah, that's why we call the update future-proofing. Because at the current moment, nothing takes advantage of it's new features (bigger payloads).

Also, Plailect's guide seems to have a rather straightforward instructions for upgrading a9lh. So, if this is harmless, why not upgrade it?

Cool. Since it's so painless, I guess I'll update it when I can actually be bothered about it. Too many games to play now haha

IIRC, you don't need the otp.bin to update.

As for your "frankenA9LH", I couldn't honestly tell. All I know is that updating stage2 without stage1 is one of the best ways to brick, but since you managed to do that, you should probably try your luck at a casino. :P (you probably were lucky that at that time the fork was not too ahead)
