What firmware are you on?
Chances are they’re ripped and packed crudely to get the content out there.

firmware is 6.72. Yes, the affected DLCs work in game.
One example is the 2 dlcs addons from Metro Exodus, that are in fact themes. They show a little picture with '?' image and the test they show is just '-'.

Sounds like the packer didn’t include everything they needed to.
Probably just need to manually ftp an icon to the adc (addcont) folders they have installed to. As for the name it’s probably the SFO or Eboot.
Alternatively open the fpkg and add them and reinstall.
do you mean whenever you install the file? if it's not in the notifications section, could be a notification.db error. you can change that with either the package or param.sfo (can't recall which), but names for certain dlc like themes are int he param.sfo. I know that for sure. I just made a fix pkg yesterday. if it's in system storage, I bet it's the addcont.db issue. you can look at that with sq lite browser. it's in the system_data folder from what I remember. @KiiWii has the location exactly. in his tutorials. I always forget it.

btw, iirc, you can build a param.sfo with no name or provider. it's in a different section. it will warn you if you don't have them, but I think it will still create the param.sfo if you have it do so.

basically, there's a way to name a package something to where it will display in notification.db while also keeping a different name for the pkg itself, but I can't recall if that's a package setting or a param.sfo one. normally, pkgs will appear in notification.db based on what you named them, but I think there's a way around that. it won't be named that way in system storage though. that's what will have the param.sfo name I believe, so it really depends on which one is messed up.

@grabman this is very strange, i have fixed the metro exodus addons browsing, just uninstalling the 2 themes that was showing, and intant off, the 2 dlcs are showed: the two colonels and Sam's story. I cant understand why is happening this because, this is not the case as you say on the video, an EP pkg that contains UP ids... this is not my case.
So... are the themes interfering with the correct working of the DLC browser ?
Other case: The last of Us remastered + update 1.10, with no themes installed and no dlcs. It is showing a DLC on the browser, with no pic and no text, just '-'. I have checked the 'addcont' folders, and nothing there about the game CUSA... so what is happening here ?

I'm assuming the themes are using the icon0.png for the icon just like they do with the preview icon. do the themes' preview icon show up under the themes section? if so, I'd imagine something weird is happening with the appcont.db. I can't recall if it does this on the ps4, but I know the app.db on the psv shows the location of the icon0.png. if it's missing, the bubble will have no icon.
oh hey man. Don't like skim the video follow along? It's really like a combo video. Cause sometimes you want to change region on dlc. Which was my scenario here. Not only did I have to change region but I had to add icons.
What you do is just ignore the change region bit. Follow the rest of the instructions.
Here is reason why icons don't show up ok. Just to try to help you because you are confused (others are confused) and it's not helping ANYONE. When you use dlc unlockers right. That's considered dlc without extra content. But the way the tools work is only the dlc with extra content gets icons. So what is solution? You make unlockers dlc with extra content the extra content being the icon.
As you correctly noted when the theme is uninstalled the icons are there (they are there in scene release makes sense) so the problem sir is how you or someone made the theme and well I am not sure what's the deal but obviously your theme is TOTALLY MADE IMPROPERLY. Good luck!
here's an unlocker for the final fantasy vii midgar theme:
as you can see, it contains no icon0.png, just the licensing stuff. I know that if you try installing a theme itself (without unlocker) it will error with no icon0.png. it will say it failed to install, but it will still be present and installable in the themes' section.
You have to install theme, fix, then theme again or the locked theme will disappear.
dunno. some dlc can be unlocked with a simple pkg just like themes. I think it's mostly on disc dlc. I'll have to check my stuff to see if I have anything like this. the names are in the param.sfo, so if nothing is listed, it will be blank in the home menu. everything requires a param.sfo or the pkg build will error immediately before it even begins. however, name and provider can be left blank. it will warn you about it, but it doesn't matter. I think whoever pkg'd those themes and dlc up didn't bother with the icon0.png or param.sfo. they ignored warnings or errors and built them anyway.
