The card will not be formatted, but the subfolder in "Nintendo 3DS" that relates to your old NAND (on the target 3DS) will be deleted. The subfolder that relates to the to-be-transferred NAND on the source 3DS will be formatted just the same.
You probably won't be using the old NAND anymore, so that's not an issue.
But if you for some reason would like to access the old NAND on either 3DS again later, make an emuNAND/sysNAND backup first (depending on what you are using, but if you're on A9LH/B9S it's probably sysNAND) then back up all the SD card files including the emuNAND/sysNAND backup to your PC and preferably to a second location as well, like Dropbox, a NAS, external HDD, whichever.
But no, you don't need to re-hack. A9LH, B9S, Luma etc are not affected by the transfer nor are any of your HBL apps. But you will need to reinstall NTR-CFW, Luma Updater, HB Launcher Loader, FBI, FreeShop (if you use those) on the target and source 3DS, along with any other CIA based homebrew you use.
Edit: The easiest way to get FBI onto a 3DS seems to be by entering Luma settings (hold Select on boot) and enabling the Service/SVC/ARM9 access patch. Then you should be able to boot FBI.3dsx from any *hax entrypoint (assuming you have one that works on your current firmware) and installing FBI.cia from there. If you don't have a working *hax entrypoint then you may have to reinject FBI with GodMode9, following the steps in Section IV from here:

You will want to make sure you have GodMode9.firm/.bin in the luma\payloads folder on SD (depending on whether you are on B9S or A9LH on that particular 3DS) before you try booting holding Start, but you should already have those if you followed 3ds.guide.
