Hardware SD card can't be used error but when reinserted works, but then doesn't work in USB loader

I have a 4gb sd card formatted with fat 16, but my wii keeps saying that the sd card cannot be used, but when I reinsert the card it works again, but once I exit and open usb loader, usb loader crashes and I reopen the SD menu and it says the card can't be used again, I used to use a 256 mb card also formatted with fat 16 just fine. WHAT IS HAPPENING!?!?

Try to format your SD card to simple FAT.

Or test another SD card.
