You can save normally or if there's a hard bit of a game that you struggle to get past then you can ust the real time save if you want to.

Yeh, it goes according to the game it would save as it usually would and I thin DSTWO also has an option to save whenever you want, correct me if Im wrong.

Correct. Games will save with a save file (.sav or .nds.sav) for each game. RTS works somewhat like a back-up, normally used when the game doesn't allow saving.

Like say, if you're playing against a boss on Super Mario Bros. and only have one life left, you can use RTS and load that back-up if you die. This will avoid you going through the whole stage again.
