I've tried this a couple months ago to have a game as the default middle r4 icon by renaming it, as you stated. It doesn't work. If there is something special you have to do give me a hit and let me know. But I'm pretty sure it doesn't work...
1) rename your _DS_MSHL.nds file to moonshell.nds and then rename whatever nds file you want to use instead to
_DS_MSHL.nds (make sure you dont have it down as a double extension eg moonshell.nds.nds or _DS_MSHL.nds.nds)
to check if you may have a double extension .... this is how you display the filename extension . .

Now you can go and rename the file extensions of any file directly
