To be honest it sounds more like you're just having adapter issues. The PSP formatting it's inserted stick is the best way to go, but if you can manage to get it recognised with the Panasonic formatter than using that on the microSDHC's first and then formatting again on the PSP (and perhaps if that's problematic Windows, then the PSP) can't do any harm.
 
The cards have to be formatted in the memory stick for it to work (unless you're only using 1 card)
Have tried formatting from windows using my card reader rather than my PSP and copying an ISO over to test now. This is the method I remember using when i first got the adapter. So should hopefully cause the least problems. Thing is that my Card Reader is slow as shit so its taking a while to copy over, though oddly it was a lot quicker to format.

I guessed it was adapter problems as, judging by Zephyr's comment, it doesn't seem to like Kingston MicroSDs (its never been able to format the stick)
 
The PSP format actually reads faster in the PSP than the Windows format apparently....

Also i initially meant....format the Micro SD's individually, stick them in the adapter and then format in Windows, stick it in the PSP then format again.
 
God, went through all that and STILL the Prometheus firmware isn't loading my 6.20+ ISOs. I'm using the Prom-4 leak (I know its probably bugger but worth a shot) so what the hell gives!?! None of my 6.20 ISOs will fit on my official memory card btw.
 
It has to be your CR 5400. There's nothing else you're doing differently to the majority of other users but that.
