I use Nova Launcher (prime), and the icon pack is called "Phlats". It's a paid icon pack. (but there should be a similar, free one, just not from this dev)
I just hid the statusbar in nova's settings and used "Zooper" widgets to make my own clock/battery thing how I wanted it.

edit: For the thread in general, mycolorscreen.com showcases some really neat android screens and most of the time the uploaders will list exactly what they used to achieve it. There's both flashy and minimalist stuff there. :3
