This is on a Samsung Epic 4G Touch, non-rooted.

The clock+date widgets came in a single pack called "Retro Clock".

Battery thing is called "BattStatt".

EDIT: Adjusted some icons on the homescreen a bit.
You need to put them in your Public folder and then right click them and choose "Copy public link", or open the Gallery view and copy the image addresses from there.

Sony Ericsson Xperia Play (Verizon)
Rooted, locked bootloader
GO Launcher with the GO Clock widget, the clock actually flips like HTC.
Fancy Widget for the weather
Statusbar+ Lite for the status bar (Unrooted phones can use this too!)
GoLocker for the Lockscreen, using the Sense theme.

Incase you're interested on how I made those two widgets look like one, I set the grid size from 4x4 to 10x10, you'll see why. After that, add the GO Clock widget and resize it to 3 rows and 10 columns. After that, go into the Fancy Widget settings, set the weather icons, locations, whatever you need, then go to Appearance Settings > Hide Widget Element, and check off everything except for the background and weather icon. Once you've done that, add the 2x1 widget, don't resize it, but put it where it is in reference to the picture. For the final step, grab the clock widget, then let go of it so it'll overlap the weather widget you just made. :)

I also have it set where if you slide your thumb upward starting on the bottom right corner of the screen, it'll load up the music player, and facebook for the left side. I like having only 3 icons on the bottom so I'm glad GO Launcher has gesture shortcuts!
 
My own creation using Nova Launcher Beta, WidgetLocker, Minimalistic text and a modified by me windows background I found. Pictures may be a bit large.​

And I finally settled for the time being on a layout on my 2month-old Rezound. It was tough to do without MIUI xD
