Clipping 3D objects.

The old man consistently trying to teach me how to catch pokemon. I hope they make it skippable, if they haven't already.
 
When its clear that your character is "sliding" around a 3D map. By that i mean look at the feet. when the planted foot goes down to make contact with the ground, it doesnt stay put. Mario Galaxy does this well, Oblivioin makes me cringe.

When your RPG party members have enough combined power to destroy the earth multiple times over by spells and summons,and a locked door stops you dead in your tracks.
 
I was going to say mosquitoes because it fits the topic.... but I would say when the opening movie of a game begins to lag.

CG "ice-skating", haha! that's always bugged me! I mean, I suppose in video games it's more excusable, but the fact that some current CG-animated TV shows still do this (like that atrocious Garfield show on Cartoon Network) is downright inexcusable!

(But, I digress!)
 
I don't like the high pitched voice of Diddy Kong in DKCR ...it doesn't sound like Diddy at all to me, and is kinda irritating..
 
That terrible "clanking" sound effect in Mega Man Legends and the later Mega Man X games that the characters make whenever they walk. Not only does it usually not sound at all like metal clanking and more like weird squeaking, but it's distracting and irritating to hear that multiple times every second whenever you're moving.
