No. Create the folder on the 3DS SD Card if it doesn't exist, 'cause that's the default .ini path.
I did, but it never creates the ini file.

The roms folder is on root, and inside is a "flashcard" folder and a "nds" folder. Then inside the flascard folder there is another nds folder.

Exactly. I still don't see why it's not working for you.

Well, there's the problem. It can't seem to access your SD Card.
Do you have "lastsave.ini" in "3dssd:/_nds/"?
I'm having the exact same problem as Boured. (but with DSTT)
No banners created, paths exist.
lastsave.ini exists:
[Save Info]
lastLoaded = fat0:/Games/rom.nds

I'm having the exact same problem as Boured.
No banners created, paths exist.
lastsave.ini exists:
[Save Info]
lastLoaded = fat0:/Games/rom.nds
It still looks like this, even when running "Add Games"? That shouldn't happen, if so.
