Gaming dead space alt-fire doesnt register

call me a noob or what but how do you do the alt-fire mode in dead space?

i tried turning the wiimote sideways but it keeps telling me to twist wiimote sideways now im stuck.
meaning sideways like when you play punch-out or dr mario?

Twist wii remote (right hand) about 90° left or right. (movement is like a opening door-lock with key)
