I just wonder, I remember in the end the levels skipped from 4-3 to 4-5. Would anyone be able to check and see if a 4-4 exist in the game code at least?

Uh oh....There is no 4-4 when I played Shadow Lair. I sense a CreepyPasta idea sparking! Im checking the other worlds if that happens too.

Snow Gate: Has 1-4 Forest Gate: Has 2-4 Beach Gate: Has 3-4. PS: In a circle of mushrooms, at night, it has words in it: MIAH+KAORI. Im searching for more of these things or about Miah and Kaori.

Every world has 4 levels before the boss. This is probably some glitch because The Rapo City gate only has 3 levels before the boss. Its maybe because since the rest of the bosses are marked (World # Here)-5, all bosses are labeled #-5.
