40mm fan (lay it a big diagonal) and screw in with 2 long screws.
then solder the + wire to 12V and the - wire to GND.

You might (if you have a fangrill) cut out a hole and 4 screw holes in the side of your 360 where the 2nd GPU heatsink resides.
Then the 40 mm fan has A LOT of room to suck in fresh air and nobody can stick their fingers into it.

I did it on my RGH box and it's noticable cooler, even when running a game for 45 minutes and it's standing somewhat enclosed.
 
Cutting a hole in the side is simple ;p

Soldering in a 40 mm fan is "simple"
The soldering points aren't that small near the power connector.

On a side note: You WON'T get banned from XBLive from installing a extra fan.
A lot of people claim that soldering stuff onto the mobo will get you banned but MS has no way of checking how much power a 360 draws.
 
To bad that I don't have a spare 40 MM fan left and that I live half way across the world otherwise I'd say send it to me to get it fixed.

I don't know the history of your Falcon jTAG or how hot it has become in it's lifetime but a RRoD can have a lot of causes (hopefully it is something not RAM/GPU/CPU/HANA related)
