Hacking 3ds homebrew apps won't work

hello, i have a few problems on my 3ds, anytime i select any of the homebrew apps, they have no icon on the top screen, and when i try to open them, i get the error tat says the card was removed and i have to reboot the system only to come back to the non-functioning apps. i dont know why this is happening, and everything is fine, the sd card works perfectly and everything is up to date, there is a little backstory to my 3ds just in case it might help,
when i first got it, i hacked it and it worked wonders, then soon it broke down, and soon died, only giving me a 3ds that could only turn on to a black screen for 5 seconds then pop and die. after about a year i had replaced the battery that seem to bulge out a little, and removed the sd card and it worked then.
i think it might be the sd card reader or something with the 3ds itself

Check if your SD card is fake, faulty, or dying.

1. Copy everything off the SD card to a computer.
2. Quadruple reformat.
3. Full Write + Verify the empty card in H2testw. Do not skip.
