To fix your issue as I had the same and for all people looking to fix;yeah the console prompted me to format the sd card. I've also done it with guiformat32 and minitool partition wizard.

failed to init sd card for CFW and BOOT.DAT? for sxos , replacing the sd card adapter in the switch will solve your issue, you do not have to buy another switch like some people have done! The adapter either becomes shorted out or the surface mount capacitor dies.
Stick to using FTP for transfer of files rather than removing the sd card every time like I did.

How long did it take to send and receive?UPDATE! 07/24/18 So turns out my switch had a weird factory defect with the sd card. It would work normally for all things official firmware related but payloads didn't have access to the sd card. The other symptom was that if i pressed down on the kickstand the switch would think the sd card was removed.

I sent it in to Nintendo for repair and they replaced the sd card reader. Now sx-os/hekate work fine.
From what i can tell from this experience is that there's no way for them to patch it once the switch leaves the factory line. The patch is applied during production not after.

just got my sx-pro and following the steps and i get this error every time with 4 differnt sd cards from sandisk 2 of 64gb, 128gb, 200gb. have tried fat32 and exfat and same result.
also added a 16gb sd to the test and same result.
Now I've sent the console to Nintendo for repair and they've serviced it and it's on it's way back. I'll let you know the results.

I think it has to do with the fact that CFWs dont support sdsc. So either the sd card reader is not connected properly or maybe you damaged part of where the sd card reader plugs in. My issue was that the sd card reader was not connected properly and would get boot.dat? but otherwise the switch would function normally under official firmware ie. reading sd card, saving etc.hi there guys im new to this forum but i think i may have discovered something.. ive recently started a service installing the trinket m0 and when i did the first one (my own) all the soldering went well and everything was looking good.. right up to the point where the metal shield was on and i was inserting the sd card. i hulk smashed the sd card reader back in and bent the pins like an absolute lemon. rather than wait ages for ebay or china to deliver me a new sd card reader.. i bought a brand new switch from argos because they have same day delivery. i nicked the sd reader out of that and stuck it in mine and i was away to the races! great!
well not so great! i then ordered a new sd reader off ebay for the second switch. but when it arrived i was constantly getting the dreaded ''Boot.dat'' screen when trying to boot into sx os. i thought the seller on ebay had sent me a dud.. because the sd card reader from my first switch would transfer over and work fine (ruling out the mainboard and my soldering) so i sent it back... i then ordered 2 more from different sellers.. and low and behold today comes and both these new ones constantly give me the boot.dat screen.
now there are differences ...all the sd readers that dont work all have circle holes in the silver part where the sd card fits.. the original does not..
the readers that dont work (oem ones found on ebay amazon aliexpress etc...) all have the same numbers printed on the circuit...HAC-SD-01 CA HF
the reader that does work and the one i hulk smashed has HAC-SD-01 TS HF
this is 100% the reason im getting the boot.dat error because the new sd readers throw up the error.. and the original doesnt.. and this is between 2 switches. but online. they all seem to be these oem variants CA HF...
i dont know if this helps anyone and im sorry i dont know if i can even include pictures! but im now on the hunt for another sd reader with TS HF written on it so i can get this second modded switch sold!

also!! all the sd card readers with holes in- the new ones (CA HF) << they do actually seemingly work fine in official firmware and it doesnt seem to matter weather its exfat or fat32! its only a problem when trying to boot into sxos

thats very interesting.. but surely i would get the same effect with all of the sd readers? i have several of the oem replacements and they all function the same on all my switches i have to hand. where as the original functions fine.. why would the oem replacements all end up in sdcs mods (im not familiar with this terminology). i can plug my original in to any switch at any time and boot either firmware easily.. the oem's all function the same.. i have now found replacements on aliexpress with TS HF on them.. so i will get back once i receive them.. my thinking is that if i would have damaged something for the sd card to run in a lower mode (im assuming this is a similar affair to pci bus lanes- again im no expert) then it should be repeatable across all my sd readers and it wouldnt be repeatable across multiple consoles? thanks for the input though.. it was puzzling me for days and i still dont understand why the oem readers wont read the boot.dat! its well annoying!!
my main point being that im not just dealing with one sd reader or one console.. i have a 2 oem sd readers from ebay.. and 2 consoles and only 1 original sd reader.. the logic in this is that there must be a difference between these two types of sd readers?

Following, I damaged my initial sd reader after doing a SwitchME mod and trying to reinsert the sd coupler. I bought a replacement off of ebay and get the dreaded "missing boot.dat" and the glorious being flicked off by my sd card. Under heavy magnification I can't see anything wrong with the connector on the main board so it almost HAS to be these non OEM card readers. If you have an update on this I would surely appreciate it.... I've already got a 2nd system that is working great with SX OS and a SwitchMe but I would really like to get this other one able to accept a payload since Nintendo has stopped producing RCM capable units.
*Edit* Further examination of the original sd-card port that I damaged is ZD HF

most likely you have a loose sd card reader. open your switch and reconnect the sd card reader and make sure it's seated properly. there are switch breakdown videos on youtube if you need help.I'm having weird issues. SXOS gives boot.dat? error 9/10 tries. Then on 10th time it boots fine. Same with nsp games on sd card. Get software cant be used error or sd cant be read errors, then randomly after umpteen attempts, the game miraculously loads. On 6.1.0 emu, 6.1.0 ofw and sx 2.3 (tested all previous sx firmwares and same problem.) Driving me crazy. Would much rather it didnt work at all than worked intermittently!! If its a fault the Switch's sd reader, surely it wouldnt work at all ... its defo not the SD card ... worked perfectly for months and still works perfect on my other Switch. Anyone else have this?
Cheers, thats what i was thinking too. It does seem to work if you tap it pretty hard so i'm guessing its an sd card reader loose ribbon connection.most likely you have a loose sd card reader. open your switch and reconnect the sd card reader and make sure it's seated properly. there are switch breakdown videos on youtube if you need help.
