If the icon is missing it sounds like the title is gone as well so you should be able to install via FBI with no problems.
If you want to be sure though, launch FBI and change the the mode to "Delete Title", then look for and delete the following titles if they exist.
CTR-N-HGRE
CTR-U-HGRE
CTR-P-HGRE
CTR-T-HGRE
Then change the destination to NAND and check for and delete them if they exist there. The only one that SHOULD exist is CTR-N-HGRE, and it should only exist in NAND, but make sure the others do not exist in either location as well.
Once all that is done you can change the mode back to "Install cia" and set the destination to NAND and install the title you extracted from the 9.5 update package.

If you are using gateway then install the updated version of the title that you downloaded from 3DNUS, but make sure its installed to NAND and not SD.

For reference, CTR refers to titles that are for the 3DS, the N designation means its a system title intended to be installed to the NAND and the HGRE is the unique identifier that is used by the eshop/tiger app.
T is for demos, P is for applications/games not intended for NAND install and U is for patches/updates.

Hello GBATEMP this is a quick tutorial on how to access the eShop and sign into or manage your NNID settings, but first let's get a few checks before doing anything.
Requirements
The Gateway Red card (CFW users check the notes below)
A N3DS / N3DSXL / N3DSLL
An Emunand partition with firmware 9.5.0-XX U/E/J

Ok with that out of the way lets begin.
Open 3DNUS as an administrator and download the NVER and TIGER for your region they are as follows

Here is an example of what you would type into 3DNUS if I were trying to download the U NVER
Once you have both the NVER and TIGER CIA's inside of the 3DNUS 1.9 folder move them both to your SD card where you normally keep your CIA's
Before you install either of those CIA's Plug your MicroSD card into your PC using an adapter and open Emunand Tool then select Extract Emunand and save emuNAND.bin to your desktop this will be a backup incase the update fails and your Emunand bricks.
Once you have a backup simply boot into your Emunand. To confirm you are in Emunand open the rom selection menu and check the top right for Emunand if it says Sysnand do not continue as can possibly brick your N3DS.
From there open Dev Menu from either your Red card or as a CIA and install both CIA's. Once completed shutdown your N3DS and boot into Emunand did your brick? If you did simply inject your Emunand backup with emuNAND Tool and try again.
Spoofing the eShop and NNID Settings
When your Enter Emunand install Free multi patcher and run it.
Your setting in it should say
e-shop spoof - on
region patch - off
no auto download - on
then click A to save and start to apply those patches
From there simply enter the eShop like you normally would or go to your system settings and open Nintendo Network ID Settings to log into your NNID, set up a NNID, or change its settings.

on other cfws and also on 10.2- sysnand its passivle to install nver and use hans for eshop access, but there is a problem downloading some titles if you use hans from the newes starterpack. Using an onlder version fixed it on both of my devices.
Also the free multi patcher is only needed for game dlc and application checking the system version, if hans is used for accessing the eshop.

Personally though I would download the latest gateway files (3.6.2) and put the launcher.dat file on your sd card and copy the /3ds/ folder over, then launch the gw menu from the homebrew menu, then format your emunand from the gw menu.
Make sure to back up all of your sd card files first because the card will be formatted.

That's wrong, you have to setup a fresh emuNAND with no NNID linked and then link you NNID there. Then restore your emuNAND backup. 022-2634 means Nintendo's servers has no link on their servers to your NNID but your NAND is logged in.

Edit: If you get a blackscreen after the message with "Home menu is preparing", just delete the content of your Nintendo 3DS folder on your SD card and boot into emunand, it should work now and no more black screen.

Second, yep, Just delete the updated tiger and install the new one. Keep in mind that, to my knowledge, Title Manager is unable to delete titles. You can use FBI to delete the title (set destination to NAND first) or just use DevMenu to install it over the existing one.
Might I suggest adding a step in the guide for CFW users to install FreeMultiPatcher, run it, and then launch the eShop normally once to ensure that they are able to sign into their NNID properly and potentially clear any ToS/EULA messages?
I think the reason people are having problem getting past those on CFW is because of the nature of the way HANS loads titles and how it prevents the system from opening another title alongside it. The old eShop will open and load just fine on CFW without HANS, the only problem is you cannot access any content pages or download anything because the URLS no longer work.
