I've seen previous threads about common errors so I've tried my best to fix those.
Here is what I've done:

- Transferred my desired WBFS to an external FAT32-formatted hard drive using Wii Backup Manager (the game is a small modification of DJ Hero 2 [SWBE52] which I've confirmed works on Dolphin, created using Wiimm's ISO Tools).
- Used the d2x cIOS installer v3.1 to install the d2x-v10beta52 base57 cIOS to slot249 rev65535 (I've also tried v10beta53-alt+base57, v10beta52+base56, and v10beta52+base58).
- Plugged the hard drive into USB port 0 (the one closer to the edge).
- Opened USBLoaderGX v3.0r1272, set the game IOS to 249, turned NAND Saves Emulation OFF, and tried to start the game.

USBLoaderGX can recognize the hard drive, display the game, and play its disc channel animation, but trying to play it results in a black screen (except when using IOS-58 base, which returned to the homebrew channel).

Here is my syscheck (ignore the system time, I haven't set it properly).

your setup is good and your steps to make it work too.

does the official game (not modded) work fine?
is it a problem only with the modded one?

Try another loader, wiiflow for example. if it works, it's an issue with a setting in USBloaderGX. if not, it could be a problem with the patch or mod itself. do you know any other users who successfully play it on real hardware? which loader and setup ?

The official game doesn't work either, in UsbLoader or WiiFlow. UsbLoader closes to the Homebrew Channel, and WiiFlow closes to the System Menu.

The original disc I ripped it from works fine, and both WBFS work fine in Dolphin.

The mod itself is custom-made by me so no others have played it. However, that seems to not be the issue at this point.

I will try some other game WBFS as well.

did you try to play the disc launched from the loader instead of the official disc channel?

Do you know the official requested IOS for that game?
lot of music games work better with base57, but that's not always true. some games require base 38 (animal crossing) or 53 (sponge bob) for example.
I think you can see the requested IOS in WiiBackupManager.
if not 56/57/58, did you try a cIOS with the official requested base?

Another thing you could try is Hermes cIOS, if you are on Wii (not vWii).
these are usually installed in slot 222, 223 and 224. 223 have a mix of two different base and was recommended a lot for music games.
note that you actually don't have them installed, you'll have to either use the installer, or modmii to get them.

The accessory is connected through USB?
be sure it's plugged before launching the game, as only d2x v10 r53 (10-alt) is supporting plug&play, but that version of the cIOS is having compatibility issues so it's not recommended to use for all games.

Ok, I managed to get it working! DJ Hero 2 uses IOS57, so I installed the same cIOS with base57. Then the official game started working but the mod was still getting a black screen.

It turns out I forgot to change the Ticket and TMD, so the Wii was blocking the improperly-signed code. After deleting them and letting Wiimm's ISO Tools generate new ones using Trucha, it started working. This is probably why it always worked on Dolphin, which seems to ignore code signing.

Thank you so much!

thank you for letting us know what you did to fix your problem :)
I wouldn't have thought about the wrong signature, usually modding tools take care of that automatically, but you said you did the mod yourself so that make sense you can forget to do it manually.
I think Wiiscuber does it automatically, Trucha signer doesn't and you need to select the option after editing a file.
