Your USB needs actually 3 folders:
USB:/ apps / usbloadergx <- here goes your dol file with icon and xml files.
USB:/ usbloader <- here goes things like game's covers and other stuff required to make your stolen games look pretty.
USB:/ wbfs / Name_of_stolen_game_[ID OF STOLEN GAME] / ID OF STOLEN GAME.wbfs <- These will be your stolen games.
Seriously dude.. there's the same question posted every 10 minutes. Is that hard to google a bit before asking an already asked question?
Of course nothing personal, but it amazes me how people can't seem to use logical thinking nowadays.
EDIT: SYSTEM VOLUME INFORMATION folder is a hidden folder that Windows creates everytime you plug in a drive. It's nothing important. Basically a folder Windows uses to talk to the drive. You could delete it if you want but it'll get restored again. Usually it's a sign of corrupted data too. So I'd format that drive if I were you.
