I wonder what's the diff between IOS249 rev 14 and CIOS249 rev 14

I have tried every possible combination I can think of and could not succeed running the game from my USB drive. What am I missing?

Usbloader GX
(Error 002 fix, PAL 50/60, NTSC, Auto and all other video settings, Language settings English and Auto, Patch video, none to all)

System menu 4.0E (ios 56-64 installed)

And two different images, scrubbed NTSC and non-scrubbed PAL.
