It all really depends. Look at Back to the Future. Two was horrible... But three wasn't THAT much better. One is just... Awesome. Halflife 2, FFTA (Technically a 'sequel' to the PS1 version), were both great 2nd iterations. Sometimes the third is better, sometimes the second, sometimes all but the first one are horrible and make you want to put your head into a meat blender and hold down the blend button.

As for the TV thing, I think generally it works the same as games -

3rd = 1st > 2nd

For example, Doctor Who (new series), the 3rd series the one with the master etc. was as good as the first, but the 2nd series was terrible.

Good job the 4th series is on now =D Watched it last night, was pretty good.

But back to games, I agree that the best Phoenix Wright game was the 3rd, then 1st then 2nd, and the 4th I don't know where to put because I constantly change my mind about it (but never lower than 2nd, although the last case of JfA was good).

Then again, I suppose sometimes the first can be the worst too

Red Dwarf

Series 3 and 4 were both brilliant

(In fact, I thought 1 + 2 both sucked, actually, too... grey and dull)
And Dr Who definitely is looking very good.
