I use wiiflow, it rips my games directly to my storage device, with no extra work. Just be sure is formatted properly and that you set to SD slot to load game from or USB to get the right location.

I know, i was just saying what i do. I wonder if the storage device he is using is defective. Use this to test it http://www.heise.de/ct/Redaktion/bo/downloads/h2testw_1.4.zip

If is defective, then is probably why you can't do it. Try something else, like a SD card in the sd card slot.
It says "Error accessing target." I can use Wii Backup Manager to add games and I can load games off of it just fine, so I don't think the drive is defective. I've also tried it with an SD card, no luck. I think that means that it's the Wii or wrong cIOS or something.

So you sound positive is the wii fault to blame. Did you do anything with it such as mess with the cios at all? Or did you buy it used or something? You may want to try and find a CIOS checker to be certain is how you think it is.

Is possible you can still play game backups with no issue right? So how can the wii be the reason why is not ripping the game? Otherwise, use syscheck to see if your ios is in check. I also found this...

I got it new about 2 years ago, then about 1 year ago I installed homebrew. First thing I did was get USB Loader GX and use the d2x cios install and just ran it with default settings, which I think was d2x v6 rev 20016 base 37 slot 249. USB Loader worked until about a week ago, when it just stopped working. I looked up how to fix it and installed how it tells you to on the CFG Loader installation guide here http://www.wiihacks.com/showthread.php?t=108557 with d2x v10 beta53 rev 21010, and that let me load games again. I was never able to rip games from the disc.

Does your disc still work like normal, are there scratches on it?

d2x v10-beta53 really isn't recommended. Use v10-beta52 or v8 instead.

Flash drives are very flaky on Wiis and writing large files to many USB devices in general seems to lock up. Your best bet it is to use CleanRip to rip them to an SD card then transfer them to your storage drive using Wii Backup Manager. If you absolutely MUST use WBFS partitioning don't use WBFS Manager. It corrupts games when adding new ones.

You still have yet to check your ios with the syscheck homebrew have you? Are you also on the latest firmware 4.3, did you read the link i posted?
I read the link, but that does not help as CleanRip isn't compatible with WBFS drives. I can do a syscheck tomorrow.

I use Wii Backup Manager. I don't have any SD cards larger than 8 gigabytes, and it's full of stuff that I can't back up. I can't get any others.
I'll try those other versions tomorrow also.

@H1B1Esquire Maybe read more than three words in my post? I said rip them to SD then transfer them.

@OP Well, I don't know what to tell you then. A 32GB SD card costs $12.88 at Walmart. :P

Again, sorry I could not get to the computer yesterday. SysCheck crashes before it can produce the log, so that's yet another thing I need help with..
