Just found this YouTube video of a small team that created an adapter for the NES that monitors the gameplay on real hardware to trigger achievements.



I think this has potential to be adopted by streamers and speedrunners to unlock a new era of tracking records and other challenges. Just like hardware based segment splitters, input display and acquired items overlays or even flashcarts with USB or networking support.
