The only things missing in cios 249 in my oppinion are:
- frag-list (for support of ntfs, fat, and other filesystems with .iso .wbfs or other formats)
- proper handling of games with ios reload problem.
If these two things are added to the monolithic version of 249 then I don't feel the need for external modules.
If you don't plan on adding these then external modules are highly wanted.

Well, as far as I know (and as tj_cool stated), normaly Homebrew that uses IOS249 just needs the Trucha-Bug, and maybe some other patches it has (DI_Verify, etc..), and not more. I doubt that the new cIOS wouldn't have these...(though, I'm just assuming...).
